Appointment Types

New Patient Evaluation (60 minutes)
Comprehensive assessment, review of history, diagnostic clarification, and development of an individualized treatment plan. Medication options are discussed when appropriate.

Follow-Up Visit (25–40 minutes)
Depending on your needs, follow-ups include medication review, symptom monitoring, brief supportive therapy, and ongoing treatment planning. You may choose the length of your follow-up when scheduling:

(1)  Standard follow-up (25 min)

(2)  Extended follow-up (45 min)

What to Expect

  1. You request an appointment

  2. We send you email confirmation with a link to intake paperwork

  3. You complete your online intake paperwork

  4. We send you a link to the patient portal & you can schedule your intake appointment

  5. After your initial appointment you will be able to self-schedule your follow-ups online

Locations

  1. Telehealth appointments are currently available for patients located in Arizona

  2. In-person appointments are available at our office in north Phoenix, AZ

  1. We are currently a cash-pay practice.

  2. You can request & submit a superbill to your insurance for out-of-network reimbursement

    Initial evaluation (60 minutes - $325) 

    Standard follow-up visit (25 minutes - $185)
